Referral to a medical oncologist would be for upfront chemotherapy. And oftentimes this is for downstaging to help us with surgery and to also prevent local recurrence, especially in the case of rectal cancer. Another reason may be for metastatic disease or stage four disease. At that point, the best step is chemotherapy with a medical oncologist, because surgery is not going to remove all of the disease. At that time, you would need chemotherapy to really attack all of the tumor cells which have now spread to multiple organs.
